Request an Appointment
Fill this out and someone from the office will call you back during business hours to confirm a time. Please don't include detailed medical information here.
This form is a request, not a confirmed appointment. Your appointment is set once someone from our office speaks with you. Please do not use this form for a dental emergency. If you have facial swelling, difficulty breathing, or difficulty swallowing, go to an emergency room. This form is not a secure channel for protected health information, so keep the details general and we'll take the rest over the phone.
Before Your First Visit
Bring your insurance card. Common questions.
Which office should I call?
Whichever is easier to get to. Fort Smith is (479) 226-3500 and Roland is (918) 503-6262. It's one practice with one set of records, so you can be seen at either.
Are you open on Friday or the weekend?
Our scheduled hours are Monday through Thursday, 8:00 a.m. to 5:00 p.m. at both offices. Call for anything outside those hours and follow the instructions on the office message.
How fast will someone respond to the appointment form?
We answer requests during business hours, Monday through Thursday. If your issue is urgent or painful, call instead of using the form.
I'm having a dental emergency right now. What do I do?
Call the nearest office as early in the day as possible and describe what's happening. We hold time for emergencies and see same-day patients when we can. Facial swelling, trouble breathing, or trouble swallowing means the emergency room, not a dental office.
What should I bring to my first appointment?
Photo ID, your insurance card, a list of medications you take, and any recent X-rays or a sleep study if you have one. If you wear a denture, night guard, or retainer, bring it. How the old one fits tells us a lot.
Can I get a treatment estimate before I commit to anything?
Yes, and you should ask for one. We verify your benefits and give you the cost in writing before treatment begins.

Four questions. Our care team calls you back within one business day. If you are in pain right now, call the office directly and we will get you seen.
